Online Scheduling and Patient Portal

You may register for a new account with your email or link the account to an existing Facebook or Google account.

To register with your email:

• Enter a password of your choice in the password space and again in confirm password. The password must contain a minimum of eight characters and include one upper-case letter, one lower-case number, and one number or symbol.
• Type in your date of birth in mm/dd/yyyy
• Read and accept the terms and conditions
• Click Register

To register with your Facebook or Google account:

• Click the Facebook or Google button to link your patient portal account to your social media account.
• If you are not already logged into your social media account, enter your credentials.
• Enter your date of birth to verify yourself as mm/dd/yyyy
• Read and accept the terms and conditions
• Click Register

Now that you are registered, you have access to:

• View or schedule your appointment
• Send and receive secure messages with our staff
• View your statement
• Make a payment
• Fill out your paperwork

To schedule your appointment:

• Click Request Appointments
• Click the dropdown arrow under the reason for visit.
• Select Overnight Study if your doctor referred you for a Polysomnogram (PSG), Split, CPAP, BIPAP or ASV study. 
• Select Take Home Sleep Test if your doctor informed you of having a nocturnal Pulse Oximetry test (screening) or Home Sleep Apnea Test (home diagnostic study).
• If you are unsure what type of study to schedule, you are welcome to call us at 801-418-9167.
• Once your appointment is scheduled, the name of your appointment will be adjusted to reflect the specific test your doctor ordered.

Schedule your appointment at one of our 3 locations.

• Click the arrows to find the next available date for your appointment. 
• Click the date and time available you would like to schedule the appointment for.
• We are unable to schedule home testing at our Spanish Fork office
• If one location is full, feel free to see if there is an earlier opening at any of the other offices.

To view and complete your patient forms prior to your appointment, click the Patient Forms icon on the upper right of the home screen.

• A list of forms for your appointment will appear. 
• Click start to open the form.
• Once you have read and completed the form, click SUBMIT.
• The next form will automatically appear. Please do the same on all the forms assigned for your appointment.
• If there is an error or any information is incomplete once a form has been submitted, our office may reject the form and request you to complete or resign the form.

Once your insurance company(s) have finished processing your visit, you may be left with a co-pay. We do our best to inform you of your co pay prior to your test. Occasionally an insurance will process a claim differently than we were quoted. 

• Click the Bills button in the upper tool bar.
• Click your balance and click make payment.
